If using ePrescribe, ensure you select the address, NPI number, and NCPDP number specific to your required location below.
Verify prescription medication name, formulation, frequency, strength, directions, quantity, DAW, ICD-10, and several refills are complete.
Verify that any relevant clinical information is included with the request.
If the patient is initiating therapy, please provide a starter or maintenance dose as required. Please indicate if the patient has already started therapy.
